A worker assisting in the assembly and manufacture of wooden roof trusses was trapped in the machine and ultimately lost his leg as a result of a safety feature that did not operate as expected. We investigated the engineering design of the safety device, determining that minimal maintenance and a key design change were contributing factors in the accident.
